Overlooking Piazza Umberto I, it was already an oratory in 1566, when the Confraternity of the Blessed Sacrament was established and later took charge of the Oratory of San Nicolò. The confraternity provided the distribution of grain and other necessities to the poor. The Church of San Nicolò was remodeled in the 17th century, when it took on its present Baroque appearance.
In the central band of the façade is a niche, specially created in 1647 by Monsignor Codebò to venerate the image of the Madonna di Reggio; the church preserves a fresco of this image, which is revealed only during the celebrations of Saint Vitus the Martyr, patron saint of Montalto, on June 15. At the center of the façade there is also a large sundial, while on the sloping sides of the pediment are displayed the coats of arms of two cardinals of the Peretti family, nephews of Sixtus V. On the outer pinnacles of the pediment, two six-pointed stars are mounted on trimonzi, recalling the coat of arms of Pope Sixtus V.
